At 34–35 weeks of pregnancy, a fetus’s facial expressions can change within just a few seconds. From one ultrasound frame to the next, it can sometimes feel as though the baby is showing distinctly different emotions—curious one moment, impatient the next, and then suddenly peaceful again.
Of course, these are only creative interpretations, not clinical conclusions.
What is truly fascinating is the ability to observe these dynamic changes in real time:
- Flexible movements of the facial muscles
- Changes in the contour of the lips
- Changes in the position of the lower facial region
- Hands positioned near the face
- The correlation between real-time 2D imaging and 3D surface imaging
These fleeting movements reflect the maturation of the facial muscles and the fetal behavioral state during the final months of pregnancy.

A 3D image can capture a single expression. But a video tells the story of how that expression changes. Dynamic observation helps us realize that the fetal face is far from still. Physiological facial expressions continually change, revealing a vivid developmental process before birth.
The most remarkable part is not trying to interpret what the baby might be thinking, but having the opportunity to witness fetal development unfolding in real time.
Sometimes, ultrasound does more than reveal anatomical structures—it also allows us to appreciate the beauty of life before birth.
